Indications:
PRESART HZ 80 mg is indicated for:
Treatment of essential hypertension
Reduction of cardiovascular morbidity in patients with atherothrombotic cardiovascular disease (e.g., coronary artery disease, stroke)
Patients with type 2 diabetes and documented target organ damage
Pharmacology:
Telmisartan: A non-peptide angiotensin II receptor blocker (ARB) that selectively blocks the AT1 receptor, leading to vasodilation and reduced blood pressure without affecting heart rate.
Hydrochlorothiazide: A thiazide diuretic that inhibits sodium and chloride reabsorption in the distal convoluted tubules, leading to increased excretion of sodium and water, thereby reducing blood volume and blood pressure.
Dosage:
Initial Therapy: 80 mg/12.5 mg once daily; may increase to 160 mg/25 mg once daily after 2 to 4 weeks if necessary.
Add-on Therapy: For patients not adequately controlled with amlodipine (or another dihydropyridine calcium channel blocker) alone or with telmisartan (or another angiotensin receptor blocker) alone.
Replacement Therapy: For patients receiving amlodipine and telmisartan from separate tablets, PRESART HZ 80 mg can replace the individual components.
Administration:
Take one tablet orally once daily, with or without food, at the same time each day.
Interaction:
Telmisartan: May interact with drugs like antacids, rifampicin, and certain statins, which can affect its absorption and increase side effects.
Hydrochlorothiazide: May interact with beta-blockers, ACE inhibitors, and other antihypertensive agents. Monitor blood pressure regularly.
Contraindications:
Hypersensitivity to telmisartan, hydrochlorothiazide, or any component of the formulation
Pregnancy and lactation
Severe hepatic impairment
Cardiogenic shock or left ventricular outflow tract obstruction
Side Effects:
Common: Dizziness, headache, peripheral edema, fatigue, nausea
Less Common: Palpitations, flushing, abdominal pain, muscle cramps
Rare: Hypotension, bradycardia, syncope, angioedema
Pregnancy & Lactation:
Pregnancy: Contraindicated, especially in the second and third trimesters due to potential fetal harm.
Lactation: Not known if excreted in human milk; consult a healthcare provider before use.
