Tiapine 100 mg is indicated for:
Treatment of acute and chronic psychoses, including schizophrenia
Treatment of manic episodes associated with bipolar disorder
Treatment of depressive episodes associated with bipolar disorder
Maintenance treatment of bipolar I disorder, in combination with a mood stabilizer
Prevention of recurrence of manic, depressive, or mixed episodes in bipolar disorder
Pharmacology:
Quetiapine Fumarate is an atypical antipsychotic agent belonging to the dibenzothiazepine class. It acts as an antagonist at multiple neurotransmitter receptors in the brain, including serotonin 5HT₁A and 5HT₂, dopamine D₁ and D₂, histamine H₁, and adrenergic α₁ and α₂ receptors. The mechanism of action is not fully understood, but it is believed to involve a combination of dopamine D₂ and serotonin 5HT₂ antagonism.
Dosage & Administration:
Schizophrenia:
Administered twice daily, with or without food.
Initial dose: 50 mg on Day 1, 100 mg on Day 2, 200 mg on Day 3, and 300 mg on Day 4.
Maintenance dose: 300–450 mg/day, adjusted based on clinical response and tolerability.
Manic Episodes in Bipolar Disorder:
Administered twice daily, with or without food.
Initial dose: 100 mg on Day 1, 200 mg on Day 2, 300 mg on Day 3, and 400 mg on Day 4.
Maintenance dose: 400–800 mg/day, adjusted based on clinical response and tolerability.
Depressive Episodes in Bipolar Disorder:
Administered once daily at bedtime, with or without food.
Initial dose: 50 mg on Day 1, 100 mg on Day 2, 200 mg on Day 3, and 300 mg on Day 4.
Maintenance dose: 300–600 mg/day, adjusted based on clinical response and tolerability.
Interaction:
Caution should be exercised when Tiapine is used concomitantly with medicines known to cause electrolyte imbalance or to increase QT interval. Co-administration of Tiapine and thioridazine or carbamazepine caused increases in the clearance of Tiapine. Co-administration of Tiapine with another microsomal enzyme inducer, phenytoin, also caused increases in the clearance of Tiapine.
Contraindications:
Quetiapine is contraindicated in patients who are hypersensitive to it.
Side Effects:
Common side effects may include somnolence, dizziness, dry mouth, weight gain, and elevated cholesterol levels. Serious side effects may include orthostatic hypotension, seizures, and tardive dyskinesia.
Pregnancy & Lactation:
The safety and efficacy of Quetiapine during human pregnancy have not been established. Therefore, Quetiapine should only be used during pregnancy if the benefits justify the potential risks. It is not known whether Quetiapine is excreted in human milk. Exercise caution when administering to nursing mothers.
