Indications:
Ranola ER 500 mg is indicated for:

  • Chronic Angina:

    • Treatment of chronic angina in patients with coronary artery disease.

    • Can be used alongside other anti-anginal medications like beta-blockers, nitrates, and calcium channel blockers.

    • Particularly beneficial for patients on high doses of amlodipine who still experience angina episodes.

Pharmacology:
Ranolazine, the active ingredient in Ranola ER 500 mg, exerts its anti-anginal and anti-ischemic effects through mechanisms that do not primarily involve reductions in heart rate or blood pressure. The exact mechanism of action is not fully understood, but it is believed to involve inhibition of the cardiac late sodium current (I_Na), which may reduce intracellular calcium overload and improve myocardial function. Additionally, Ranolazine prolongs the QT interval due to inhibition of the rapid delayed rectifier potassium current (I_Kr), which extends the ventricular action potential.

Dosage & Administration:

  • Initial Dose:

    • 500 mg orally twice daily.

  • Maintenance Dose:

    • May be increased to 1000 mg orally twice daily based on clinical response.

  • Administration:

    • Take with or without food.

    • Swallow tablets whole; do not crush, break, or chew.

    • Maximum recommended daily dose: 2000 mg (1000 mg twice daily).

Side Effects:
Common side effects may include:

  • Cardiac:

    • Bradycardia, palpitations.
