Indications:MYOCARD 2.5 MG is indicated for the treatment of mild to moderate hypertension (high blood pressure), management of angina pectoris (chest pain), and in the management of stable chronic heart failure. It may be used alone or in combination with other antihypertensive agents.
Pharmacology:MYOCARD 2.5 MG contains Bisoprolol Fumarate, which is a highly selective beta-1 (β1) adrenergic receptor blocking agent.It selectively blocks β1-adrenergic receptors primarily in the heart and vascular smooth muscle, leading to a reduction in heart rate, cardiac output, and both systolic and diastolic blood pressure.Unlike some other beta-blockers, Bisoprolol has minimal effect on β2 receptors, thus generally not influencing airway resistance and β2-mediated metabolic effects.
Dosage:
For hypertension: The usual starting dose is 2.5 mg once daily. If the response to 2.5 mg is inadequate, the dose may be increased to 5 mg and then, if necessary, to 10 mg or up to a maximum of 20 mg once daily.
For heart failure: Initially 1.25 mg once daily (in the morning) for 1 week. If well tolerated, the dose can be gradually increased to 2.5 mg, then 3.75 mg, 5 mg, 7.5 mg, and finally 10 mg once daily at weekly or bi-weekly intervals, based on patient tolerability and response. The maximum recommended dose is 10 mg daily.
Dosage adjustment may be necessary in patients with renal or hepatic impairment.
Administration:
MYOCARD 2.5 MG tablets should be taken in the morning, with or without food. They should be swallowed whole with liquid and should not be chewed. Do not stop taking this medication suddenly without consulting your doctor, as this may worsen your condition.
Interaction:Bisoprolol may interact with various medications. Concomitant use with other beta-blocking agents is generally not recommended. Caution should be exercised when used with:
Calcium antagonists (e.g., verapamil, diltiazem) as they may enhance the negative inotropic and chronotropic effects.
Centrally-acting antihypertensive drugs (e.g., clonidine) may worsen heart rate and blood pressure upon withdrawal.
Class I antiarrhythmic drugs (e.g., disopyramide, quinidine) may increase AV conduction time and negative inotropic effect.
Insulin and oral antidiabetic drugs may increase the blood sugar lowering effect and mask symptoms of hypoglycemia.
Non-steroidal anti-inflammatory drugs (NSAIDs) may reduce the antihypertensive effect of bisoprolol.
Certain pain killers, medicines for asthma or blocked nose, and medicines for certain eye disorders (e.g., glaucoma) may also interact.
Always inform your doctor about all medications you are taking.
Contraindications:
MYOCARD 2.5 MG is contraindicated in patients with:
Known hypersensitivity to Bisoprolol or any components of the formulation.
Acute heart failure or during episodes of heart failure decompensation requiring intravenous inotropic therapy.
Cardiogenic shock.
Second or third-degree AV block (without a pacemaker).
Sick sinus syndrome.
Sinoatrial block.
Symptomatic bradycardia (heart rate less than 50 beats per minute).
Feeling of coldness or numbness in the extremities.
Bradycardia (slow heart rate).
Hypotension (low blood pressure).
Uncommon side effects may include:
Sleep disturbances, depression, anxiety.
Bronchospasm (difficulty breathing) in patients with asthma or history of obstructive airway disease.
Muscle weakness or cramps.
Rare side effects may include:
Nightmares, hallucinations.
Decreased tear secretion.
Hearing disorders.
Hepatitis, increased liver enzymes.
Allergic reactions (e.g., itching, rash).
Serious side effects such as severe bradycardia, worsening of heart failure, or severe allergic reactions should be reported to a doctor immediately.
Pregnancy & Lactation:Pregnancy Category C. There are no adequate and well-controlled studies in pregnant women. Bisoprolol Fumarate should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.It may cause harm to the unborn child, including premature birth, miscarriage, low blood sugar, and reduced heart rate in the child.Small amounts of bisoprolol fumarate have been detected in the milk of lactating rats. It is not known whether this drug is excreted in human milk. Therefore, caution should be exercised when bisoprolol fumarate is administered to nursing women. Consult your doctor before use during breastfeeding.
Storage Conditions:
Store in a cool, dry place away from direct sunlight and heat. Keep out of reach of children. Store oral tablets at room temperature as recommended on the label, typically between 20°C to 25°C (68°F to 77°F).
