Patients not adequately controlled with monotherapy
Olmevas HZ is a fixed-dose combination of two antihypertensive agents:
Olmesartan Medoxomil: An angiotensin II receptor blocker (ARB) that relaxes blood vessels by blocking the action of angiotensin II, thereby lowering blood pressure.
Hydrochlorothiazide: A thiazide diuretic that helps prevent the body from absorbing too much salt, which can cause fluid retention. It promotes the excretion of sodium and water, reducing blood volume and pressure.
Adults: The usual starting dose is one tablet (20 mg olmesartan medoxomil and 12.5 mg hydrochlorothiazide) once daily.
Dose Adjustment: Depending on the blood pressure response, the dose may be increased at intervals of 2-4 weeks to two tablets (40 mg/25 mg) once daily.
Maximum Dose: 40 mg olmesartan medoxomil and 25 mg hydrochlorothiazide once daily.
Take orally once daily with or without food.
Swallow the tablet whole with water; do not chew or crush.
Take the medication at the same time each day to maintain consistent blood levels.
Potassium Supplements and Potassium-Sparing Diuretics: Concurrent use may increase serum potassium levels.
NSAIDs: May reduce the antihypertensive effect and increase the risk of renal impairment.
Lithium: Increased risk of lithium toxicity; monitor serum lithium levels.
Alcohol: May enhance the hypotensive effect; caution is advised.
Hypersensitivity to olmesartan, hydrochlorothiazide, or any component of the formulation.
Anuria (inability to urinate).
Severe renal impairment.
Pregnancy, especially in the second and third trimesters.
Common: Dizziness, headache, fatigue, nausea, and increased urination.
Less Common: Hypotension, electrolyte imbalance, muscle cramps, and rash.
Serious: Renal dysfunction, severe hypotension, and hypersensitivity reactions.
Pregnancy: Use is contraindicated, especially during the second and third trimesters, due to the risk of fetal harm.
Lactation: It is unknown whether olmesartan is excreted in human milk; hydrochlorothiazide is excreted. Caution is advised; consult a healthcare provider.
Monitor renal function and electrolytes periodically.
Use with caution in patients with hepatic or renal impairment.
Avoid use in patients with severe renal dysfunction.
Caution is advised when driving or operating machinery due to possible dizziness.
Angiotensin II receptor blockers (ARBs) and thiazide diuretics combination.
Store in a cool, dry place below 30°C.
Protect from light and moisture.
Keep out of reach of children.
