SPIROCARD PLUS 20/50 is indicated for the treatment of hypertension (high blood pressure) and edema (fluid retention) associated with conditions such as congestive heart failure, liver cirrhosis, and nephrotic syndrome. It combines two types of diuretics—Furosemide (20 mg), a loop diuretic, and Spironolactone (50 mg), a potassium-sparing diuretic—offering effective diuresis while minimizing potassium loss.


Pharmacology:

  • Furosemide acts on the loop of Henle in the kidneys, inhibiting sodium and chloride reabsorption, leading to rapid and significant diuresis (fluid loss).

  • Spironolactone is an aldosterone antagonist that acts on the distal tubules of the kidney, promoting sodium and water excretion while conserving potassium.
    Together, they reduce fluid overload and control blood pressure while preventing electrolyte imbalances, especially potassium depletion.


Dosage:

  • The usual adult dose is one tablet once or twice daily, or as directed by a physician.

  • Dose adjustments depend on the patient's response, underlying conditions, and potassium levels.

  • Patients with severe liver or kidney disease may require closer monitoring.


Administration:

  • Take the tablet orally with food to reduce gastrointestinal discomfort.

  • Swallow whole with water, preferably at the same time(s) each day.

  • Avoid taking the medication late in the evening to reduce nighttime urination.


Interaction:

  • Potassium supplements, ACE inhibitors, ARBs, or NSAIDs may increase the risk of hyperkalemia.

  • Antihypertensives may have an additive effect, increasing the risk of low blood pressure.

  • Lithium, digoxin, and diabetic medications may have altered effects when taken with diuretics.

  • Always inform your doctor about all concurrent medications.


Contraindications:

  • Hypersensitivity to furosemide, spironolactone, or sulfonamides.

  • Anuria (no urine output), acute renal failure, hyperkalemia, or Addison's disease.

  • Severe electrolyte imbalances not corrected before treatment.


Side Effects:
Common side effects include:

  • Dizziness, dehydration, low blood pressure

  • Increased urination, muscle cramps

  • Nausea, vomiting, headache

  • Electrolyte disturbances (especially potassium and sodium imbalance)
    Serious side effects: irregular heartbeat, signs of high potassium (muscle weakness, slow pulse), allergic reactions.


Pregnancy & Lactation:

  • Pregnancy: Use only if clearly necessary and prescribed. Spironolactone may cross the placenta.

  • Lactation: Caution is advised; spironolactone and furosemide may be excreted in breast milk. Consult with a physician.
