T4 25 MCG is indicated for the treatment of hypothyroidism—a condition where the thyroid gland does not produce enough thyroid hormone. It is also used to help manage goiter, thyroid cancer, and as part of hormone suppression therapy. It restores normal metabolic activity and helps maintain physical and mental health by replacing deficient levels of the thyroid hormone thyroxine (T4).


Pharmacology:
T4 25 MCG contains Levothyroxine Sodium, a synthetic form of the natural thyroid hormone thyroxine (T4). It gets converted into the active form triiodothyronine (T3) in the body, which regulates metabolism, growth, and development. Levothyroxine acts by binding to thyroid hormone receptors in the nucleus of cells, controlling gene expression and protein synthesis, especially in tissues such as the liver, heart, and kidneys.


Dosage:

  • Initial dose (for adults with hypothyroidism): 25–50 mcg daily.

  • May be increased by 25–50 mcg every 2–4 weeks based on TSH levels.

  • Elderly or cardiac patients: Start with lower doses (12.5–25 mcg/day).

  • Take the medicine at the same time each day, preferably in the morning on an empty stomach.

  • Dosage must be individualized and monitored through thyroid function tests.


Administration:

  • Take orally on an empty stomach, 30–60 minutes before breakfast.

  • Swallow the tablet whole with a full glass of water.

  • Do not take with iron, calcium, or antacids within 4 hours of the dose.


Interaction:

  • Decreased absorption with calcium, iron, antacids, sucralfate.

  • May enhance effects of warfarin and increase bleeding risk.

  • Reduced efficacy when taken with carbamazepine, phenytoin, or rifampin.

  • Diabetes medications may need adjustment due to changes in glucose metabolism.


Contraindications:

  • Untreated thyrotoxicosis

  • Adrenal insufficiency (unless adequately treated)

  • Known hypersensitivity to levothyroxine or any inactive ingredients

  • Acute myocardial infarction or uncontrolled heart disease (use with extreme caution)


Side Effects:

  • Overdose may lead to hyperthyroid symptoms: palpitations, weight loss, insomnia, anxiety, heat intolerance

  • Long-term over-replacement may cause osteoporosis or heart rhythm disturbances

  • Rare: allergic reactions like rash or swelling


Pregnancy & Lactation:

  • Pregnancy: Safe and essential in pregnancy. Dose adjustment may be needed due to increased requirements.

  • Lactation: Considered safe. Monitor infant for signs of thyroid dysfunction if mother is on high doses.
