T4 50 MCG is indicated for the treatment of hypothyroidism, where the body does not produce enough thyroid hormone. It is also used in thyroid hormone replacement therapy post-thyroidectomy, for goiter management, and in thyroid suppression therapy. This medicine helps restore normal metabolism, energy, and body function.
Pharmacology:
T4 50 MCG contains Levothyroxine Sodium, a synthetic form of the naturally occurring thyroid hormone thyroxine (T4). It is metabolized to triiodothyronine (T3) in peripheral tissues. These hormones regulate the metabolic rate of tissues, affecting protein synthesis, glucose and lipid metabolism, thermogenesis, and growth and development. It replaces or supplements the body’s natural hormone production.
Dosage:
Adults (initial dose): Typically 25–50 mcg daily, adjusted every 2–4 weeks based on TSH levels.
Maintenance dose: Generally ranges from 50–200 mcg/day depending on age, weight, and severity of deficiency.
Elderly or cardiac patients: Start low (12.5–25 mcg/day) and increase gradually.
Take consistently on an empty stomach, preferably in the morning.
Administration:
Take orally with a full glass of water 30 to 60 minutes before breakfast.
Avoid calcium, iron supplements, antacids, or soy products within 4 hours of taking the medicine.
Consistency in timing improves efficacy.
Interaction:
Reduces absorption: Iron, calcium, aluminum-containing antacids, sucralfate.
Increases effect: Warfarin (increased bleeding risk), catecholamines.
Reduced effect by: Carbamazepine, phenytoin, rifampin, estrogen-containing contraceptives.
Monitor TSH and free T4 regularly when taken with interacting medications.
Contraindications:
Untreated thyrotoxicosis
Uncorrected adrenal insufficiency
Hypersensitivity to levothyroxine or excipients
Recent myocardial infarction (use with caution)
Side Effects:
If over-replacement occurs (i.e., too much hormone), symptoms may include:
Palpitations, nervousness, insomnia
Weight loss, heat intolerance, excessive sweating
Menstrual irregularities
Long-term excess may lead to osteoporosis or cardiac arrhythmias
Allergic reactions are rare but may occur
Pregnancy & Lactation:
Pregnancy: Safe and essential—requirements may increase during pregnancy
Lactation: Excreted in breast milk in small amounts; considered safe at normal doses
Close monitoring is advised during both periods
