TELMIVAS 40 MG is indicated for:
Treatment of hypertension (high blood pressure)
Reducing cardiovascular risk in patients unable to take ACE inhibitors
Protection of renal function in hypertensive type 2 diabetic patients with proteinuria
It helps reduce the risk of stroke, heart attack, and kidney failure in high-risk patients.
Pharmacology:
Telmisartan is an angiotensin II receptor antagonist (ARB) that specifically blocks the AT1 receptor, preventing the vasoconstrictor and aldosterone-secreting effects of angiotensin II. This results in vasodilation, reduction in blood pressure, and decreased workload on the heart.
Telmisartan has a long half-life (approximately 24 hours), allowing for once-daily dosing and consistent blood pressure control.
Dosage:
Initial dose (adults): 40 mg once daily
May be increased to 80 mg once daily depending on patient response
Can be used alone or with other antihypertensives (e.g., hydrochlorothiazide)
No dosage adjustment generally needed in elderly or patients with mild-to-moderate renal impairment
Administration:
Take orally once daily with or without food
Try to take at the same time each day for best results
Continue taking even if you feel well, as high blood pressure often has no symptoms
Interaction:
Increased risk of hyperkalemia with potassium supplements or potassium-sparing diuretics
NSAIDs may reduce antihypertensive effect and impair kidney function
Lithium levels may increase—monitor closely
Use caution when combining with other blood pressure medications to avoid hypotension
Contraindications:
Hypersensitivity to Telmisartan or any excipient
Pregnancy, especially 2nd and 3rd trimesters (can harm or kill fetus)
Severe hepatic impairment
Use with aliskiren in patients with diabetes or renal impairment
Side Effects:
Common:
Dizziness, low blood pressure (hypotension), fatigue
Back pain, sinusitis
Less common:
Hyperkalemia, renal impairment, bradycardia
Rare: angioedema, liver dysfunction
Regular monitoring of blood pressure, electrolytes, and renal function is advised during long-term use.
Pregnancy & Lactation:
Pregnancy: Contraindicated. Telmisartan may cause fetal injury or death if used during 2nd or 3rd trimester.
Lactation: Not recommended; insufficient data available—either stop breastfeeding or discontinue the drug.
