Indications:
TELMIVAS AM 40/5 MG is indicated for the treatment of essential hypertension, especially in patients whose blood pressure is not adequately controlled by either Telmisartan or Amlodipine monotherapy. It also helps reduce the risk of cardiovascular events such as heart attacks and strokes in hypertensive patients with other risk factors.
Pharmacology:
This medication is a fixed-dose combination of two antihypertensive agents:
-
Telmisartan is an angiotensin II receptor blocker (ARB) that prevents the narrowing of blood vessels by blocking the effects of angiotensin II, thereby lowering blood pressure.
-
Amlodipine is a calcium channel blocker that relaxes the smooth muscle in the blood vessel walls, allowing easier blood flow and reducing blood pressure.
Together, they provide a complementary mechanism to lower blood pressure more effectively than either drug alone.
Dosage:
The usual recommended dose is one tablet of TELMIVAS AM 40/5 MG once daily. The dosage may be adjusted by the physician depending on the patient's response.
Do not change your dosage without medical advice.
Administration:
-
Take TELMIVAS AM 40/5 MG orally once daily, preferably at the same time each day.
-
The tablet may be taken with or without food.
-
Swallow the tablet whole with water—do not chew or crush it.
Interaction:
-
Concomitant use with potassium supplements or potassium-sparing diuretics may increase serum potassium levels.
-
NSAIDs may reduce the antihypertensive effect of Telmisartan.
-
Caution should be exercised when used with other blood pressure-lowering agents, as the effects may be additive.
-
Grapefruit juice may increase the effect of Amlodipine.
Always consult your doctor or pharmacist about all the medications you are taking.
Contraindications:
-
Known hypersensitivity to Telmisartan, Amlodipine, or any component of the formulation.
-
Severe hepatic impairment or biliary obstructive disorders.
-
Cardiogenic shock or unstable angina.
-
Pregnancy (especially second and third trimesters).
Side Effects:
Common side effects may include dizziness, swelling (edema), flushing, and fatigue.
Less common but serious side effects include hypotension, increased potassium levels, kidney dysfunction, and allergic reactions.
Seek immediate medical attention if you experience symptoms like facial swelling, difficulty breathing, or chest pain.
Pregnancy & Lactation:
-
Pregnancy: Not recommended during pregnancy, particularly during the second and third trimesters due to the risk of fetal toxicity.
-
Lactation: It is not known whether Telmisartan is excreted in human milk; caution is advised. Amlodipine may pass into breast milk. Discuss with a healthcare provider before breastfeeding.
Therapeutic Class:
Combination antihypertensive agents (Angiotensin II receptor blocker + Calcium channel blocker)
