Indications:
Bisopro-A is indicated for the treatment of essential hypertension, particularly in patients requiring combination therapy with a beta-blocker (Bisoprolol) and a calcium channel blocker (Amlodipine). It is useful in reducing blood pressure and lowering the risk of cardiovascular events like stroke or heart attack.
Pharmacology:
Bisoprolol is a cardioselective beta-1 blocker that decreases heart rate and cardiac output, thereby lowering blood pressure.
Amlodipine is a dihydropyridine calcium channel blocker that causes vascular smooth muscle relaxation and reduces peripheral vascular resistance.
Dosage & Administration:
The usual dose is one tablet once daily, preferably in the morning.
May be taken with or without food.
Dosage should be adjusted based on the patient’s response and tolerability.
Tablet should be swallowed whole with water.
Interaction:
Bisoprolol: May interact with other antihypertensives, insulin, digoxin, and calcium channel blockers.
Amlodipine: May interact with CYP3A4 inhibitors (e.g., diltiazem), simvastatin, and other vasodilators.
Concomitant use with alcohol may enhance blood pressure-lowering effects.
Contraindications:
Hypersensitivity to any component
Severe hypotension
Cardiogenic shock
Advanced heart block or sick sinus syndrome without a pacemaker
Acute heart failure or uncontrolled decompensated heart failure
Side Effects:
Common: Dizziness, fatigue, bradycardia, peripheral edema, headache, flushing
Serious (rare): Worsening heart failure, AV block, hypotension, allergic reaction
Pregnancy & Lactation:
Not recommended during pregnancy unless clearly necessary. Bisoprolol may reduce placental perfusion. Both components should be used with caution during lactation.
Precautions & Warnings:
Monitor blood pressure and heart rate regularly.
Taper dosage gradually before discontinuation to avoid rebound hypertension.
Use with caution in patients with asthma, diabetes, or renal/hepatic impairment.
Avoid abrupt withdrawal, especially in ischemic heart disease.
