Indications:
Carvista is indicated for:

  • Mild to moderate chronic heart failure (in combination with ACE inhibitors, diuretics, and digitalis)

  • Hypertension (high blood pressure)

  • Left ventricular dysfunction following myocardial infarction to improve survival

Pharmacology:
Carvedilol is a non-selective beta-adrenergic blocker and alpha-1 blocker. It reduces cardiac output and peripheral vascular resistance by blocking beta-1, beta-2, and alpha-1 receptors. This dual action decreases heart rate and dilates blood vessels, thereby reducing blood pressure and myocardial workload.

Dosage & Administration:

  • Heart failure: Start with 3.125 mg twice daily for 2 weeks. If tolerated, dose may be doubled every 2 weeks to a maximum of 25 mg twice daily.

  • Hypertension: Initial dose is 6.25 mg twice daily; may be increased to 25 mg twice daily if needed.

  • Should be taken with food to slow absorption and reduce risk of orthostatic hypotension.

Interaction:

  • May enhance the effect of other antihypertensives and digoxin

  • Risk of bradycardia or AV block with calcium channel blockers (e.g., verapamil, diltiazem)

  • May mask hypoglycemia symptoms in diabetics

  • Use with caution when combined with CYP2D6 inhibitors (e.g., fluoxetine)

Contraindications:

  • Bronchial asthma or related bronchospastic conditions

  • Second- or third-degree AV block

  • Severe bradycardia or cardiogenic shock

  • Decompensated heart failure requiring IV inotropes

  • Severe liver impairment

Side Effects:

  • Common: Dizziness, fatigue, hypotension, bradycardia, weight gain, diarrhea

  • Less common: Hyperglycemia, visual disturbances, dyspnea, depression

Pregnancy & Lactation:
Use only if clearly needed during pregnancy; may affect fetal circulation. Caution is advised in lactating women as it's unknown whether Carvedilol passes into breast milk.

Precautions & Warnings:

  • Monitor blood pressure, heart rate, blood glucose, and renal function during therapy

  • Do not stop abruptly; taper gradually to avoid rebound effects

  • Use with caution in patients with diabetes, COPD, or liver dysfunction
