Indications:
Telmipres is indicated for:
Treatment of essential hypertension
Reduction of cardiovascular morbidity in patients with atherothrombotic cardiovascular disease (history of coronary heart disease or stroke) or type 2 diabetes mellitus with documented target organ damage
Pharmacology:
Telmisartan is an angiotensin II receptor antagonist (ARB) that selectively blocks the binding of angiotensin II to the AT1 receptor. This action prevents vasoconstriction and aldosterone secretion, thereby lowering blood pressure. It does not inhibit ACE and does not cause bradykinin-mediated cough.
Dosage & Administration:
Adults: 40–80 mg once daily, with or without food
Dose adjustment may be needed based on clinical response
Can be used alone or in combination with other antihypertensives
Swallow the tablet whole with water at the same time daily
Interaction:
May enhance the effect of other antihypertensives
NSAIDs may reduce the antihypertensive effect
Risk of hyperkalemia increases with potassium supplements or potassium-sparing diuretics
Caution when used with lithium and digoxin
Contraindications:
Hypersensitivity to Telmisartan
Pregnancy (especially second and third trimester)
Biliary obstructive disorders or severe hepatic impairment
Side Effects:
Common: Dizziness, fatigue, hypotension, back pain
Less common: Hyperkalemia, renal impairment, gastrointestinal upset
Rare but serious: Angioedema, liver dysfunction
Pregnancy & Lactation:
Contraindicated in pregnancy, especially after the first trimester due to fetal toxicity. Not recommended during lactation; alternative therapy should be considered.
Precautions & Warnings:
Monitor blood pressure, renal function, and serum potassium regularly
Use cautiously in patients with renal artery stenosis, liver impairment, or volume depletion
Avoid abrupt discontinuation
