Indications:
Pantonix is indicated for:
Gastroesophageal Reflux Disease (GERD): 40 mg once daily for 4–8 weeks.
Gastric Ulcer: 40 mg once daily for 4–8 weeks.
Duodenal Ulcer: 40 mg once daily for 2–4 weeks.
Helicobacter pylori-associated Duodenal Ulcer: 40 mg twice daily for 1 week in combination with Amoxicillin and Clarithromycin.
Zollinger-Ellison Syndrome: 40 mg once daily; dosage may be adjusted based on clinical response.
NSAID-induced Gastric or Duodenal Ulcer: 20 mg daily for long-term NSAID users.
Pharmacology:
Pantoprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion by irreversibly binding to the H+/K+ ATPase enzyme system at the secretory surface of the gastric parietal cell. This action leads to inhibition of both basal and stimulated gastric acid secretion, irrespective of the stimulus, and persists longer than 24 hours.
Dosage & Administration:
Adults: 40 mg once daily, preferably in the morning, with or without food.
Duration: Typically 2–8 weeks, depending on the condition being treated.
Administration: Swallow the tablet whole; do not chew or crush.
Interaction:
May interact with drugs such as Atazanavir, Nelfinavir, Methotrexate, Warfarin, and certain antibiotics.
Avoid concurrent use with other PPIs or H2-receptor antagonists.
Contraindications:
Hypersensitivity to Pantoprazole or any component of the formulation.
Severe liver impairment.
Concurrent use with certain drugs (e.g., Atazanavir).
Side Effects:
Common: Headache, diarrhea, nausea, abdominal pain, flatulence.
Serious: Liver dysfunction, hypomagnesemia, vitamin B12 deficiency, Clostridium difficile-associated diarrhea.
Pregnancy & Lactation:
Pregnancy: Category B; use during pregnancy only if clearly needed.
Lactation: Excreted in breast milk; use with caution.
Precautions & Warnings:
Long-term use may lead to bone fractures, kidney disease, and low magnesium levels.
Monitor liver function and serum magnesium levels during prolonged therapy.
Discontinue if signs of Clostridium difficile-associated diarrhea occur.
