Indications:
Ramoril is indicated for the treatment of hypertension, heart failure (especially after myocardial infarction), and to reduce the risk of cardiovascular events in high-risk patients such as those with diabetes or coronary artery disease.

Pharmacology:
Ramipril is a prodrug that is converted in the liver to ramiprilat, its active form. It inhibits the angiotensin-converting enzyme (ACE), preventing the conversion of angiotensin I to angiotensin II, resulting in vasodilation and reduced blood pressure. It also decreases aldosterone secretion, leading to reduced sodium and water retention.

Dosage & Administration:
The usual starting dose is 1.25 mg once daily. Dosage may be increased gradually based on clinical response and tolerance. It may be taken with or without food, preferably at the same time each day.

Interaction:
Concomitant use with potassium supplements, potassium-sparing diuretics, NSAIDs, lithium, and other antihypertensives may lead to serious interactions. Monitor renal function and serum potassium.

Contraindications:
Contraindicated in patients with a history of angioedema related to ACE inhibitors, bilateral renal artery stenosis, and during pregnancy. Not recommended for use in combination with aliskiren in diabetic patients.

Side Effects:
Common side effects include dry cough, dizziness, headache, fatigue, hypotension, and elevated potassium levels. Rare but serious side effect is angioedema.

Pregnancy & Lactation:
Contraindicated in pregnancy (especially in the 2nd and 3rd trimesters) due to risk of fetal injury or death. Use with caution in lactating mothers only if benefits outweigh risks.

Precautions & Warnings:
Monitor renal function and electrolytes regularly. Caution in patients with impaired kidney function, low sodium levels, or volume depletion. Sudden discontinuation should be avoided.

ACE (Angiotensin-Converting Enzyme) inhibitors.
