Indications:
NOBESIT XR 500 is indicated as an adjunct to diet and exercise to improve glycemic control in patients with type 2 diabetes mellitus, especially in overweight individuals.
Pharmacology:
Metformin decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization. The extended-release formulation allows slow release for prolonged effect.
Dosage & Administration:
Adults: Starting dose is usually 500 mg once daily with the evening meal.
Dose may be gradually increased based on blood glucose response, up to a maximum of 2000 mg per day.
Take with food to reduce gastrointestinal side effects. Do not crush or chew the tablet.
Interaction:
Caution with alcohol, iodinated contrast agents, and other antidiabetic drugs. May interact with beta-blockers, diuretics, corticosteroids, and ACE inhibitors.
Contraindications:
Renal impairment (eGFR <30 mL/min/1.73 m²)
Metabolic acidosis, including diabetic ketoacidosis
Severe hepatic impairment
Hypersensitivity to Metformin
Side Effects:
Common: Nausea, vomiting, diarrhea, abdominal pain, loss of appetite.
Rare but serious: Lactic acidosis (life-threatening).
Pregnancy & Lactation:
Generally considered safe during pregnancy if necessary. Should be used during lactation only if prescribed by a physician.
Precautions & Warnings:
Regular monitoring of renal function is essential. Use with caution in elderly and patients undergoing radiologic studies involving contrast agents. Avoid excessive alcohol intake.
