Indications:
Lopirel is indicated for:
Prevention of atherothrombotic events in patients with recent myocardial infarction (heart attack), stroke, or peripheral arterial disease
Used in combination with aspirin in acute coronary syndrome
Prevention of stent thrombosis after coronary stenting procedures
Pharmacology:
Clopidogrel is a prodrug that is metabolized in the liver to produce an active metabolite which irreversibly inhibits the P2Y12 component of ADP receptors on the platelet surface. This prevents platelet aggregation and clot formation.
Dosage & Administration:
Standard dose: 75 mg once daily, with or without food
A loading dose (usually 300 mg) may be given in acute settings
Take at the same time every day
Do not stop suddenly without medical advice
Interaction:
Increased bleeding risk with anticoagulants (e.g., warfarin), NSAIDs, and SSRIs
Proton pump inhibitors (omeprazole) may reduce Clopidogrel activation
Caution with drugs affecting liver enzymes (e.g., CYP2C19 inhibitors)
Contraindications:
Hypersensitivity to Clopidogrel or any component of the product
Active bleeding such as peptic ulcer or intracranial hemorrhage
Severe liver impairment
Side Effects:
Common: Bleeding, bruising, diarrhea, abdominal pain
Less common: Rash, headache
Serious but rare: Thrombotic Thrombocytopenic Purpura (TTP), severe allergic reactions
Pregnancy & Lactation:
Should be used in pregnancy only when clearly necessary. It is unknown whether Clopidogrel passes into breast milk—use with caution during lactation.
Precautions & Warnings:
Inform doctors before surgery or dental procedures
Monitor for signs of unusual bleeding
Use caution in patients with risk factors for bleeding (e.g., trauma, GI ulcer)
