🔸 Indications:
Sitagil M 50/500 ER is indicated for:
-
Management of type 2 diabetes mellitus in adults
-
Used when diet, exercise, and single-drug therapy do not provide adequate control
-
Can be used as initial therapy or in addition to other antidiabetic medications
🔸 Pharmacology:
Sitagil M combines two antidiabetic agents:
-
Sitagliptin: A DPP-4 inhibitor that increases levels of incretin hormones, thereby improving insulin release and reducing glucagon secretion.
-
Metformin ER: A biguanide that decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity.
Together, they act synergistically to improve glycemic control without causing significant hypoglycemia.
🔸 Dosage:

Usual adult dose: 1 tablet once or twice daily with meals (as prescribed)

Dose should be individualized based on effectiveness and tolerability
🔸 Administration:
Take orally with food to reduce gastrointestinal side effects. Swallow whole, do not crush or chew. Follow consistent daily schedule.
🔸 Interaction:

Cationic drugs (e.g., amiloride, digoxin) may compete with Metformin
-
Alcohol increases the risk of lactic acidosis
-
Beta-blockers may mask symptoms of hypoglycemia
-
May interact with insulin and sulfonylureas to increase hypoglycemia risk
🔸 Contraindications:

Renal impairment (eGFR < 30 ml/min/1.73 m²)

Acute or chronic metabolic acidosis, including diabetic ketoacidosis

Hypersensitivity to Sitagliptin or Metformin

Severe hepatic impairment
🔸 Side Effects:

Common: Nausea, diarrhea, abdominal pain, headache

Rare but serious: Lactic acidosis, pancreatitis, allergic reactions

Sitagliptin: Rare reports of joint pain and nasopharyngitis
Seek immediate help for symptoms like unusual fatigue, rapid breathing, or severe abdominal pain.
🔸 Pregnancy & Lactation:

Use during pregnancy only if clearly necessary and prescribed

Not recommended during breastfeeding without physician consultation
🔸 Precautions & Warnings:

Assess renal function before starting and periodically during treatment

Monitor for signs of lactic acidosis

Discontinue temporarily before procedures involving contrast dye or major surgery

Use with caution in elderly patients and those with heart failure
