Indications:
Cilny 5 MG Tablet is indicated for:
Management of hypertension (high blood pressure)
Prevention of angina pectoris (chest pain due to heart-related issues)
Reducing the risk of heart attacks and strokes in hypertensive patients
Pharmacology:
Cilnidipine is a dihydropyridine calcium channel blocker that inhibits L-type and N-type calcium channels. This dual action leads to vasodilation, reducing peripheral vascular resistance and blood pressure. It also decreases sympathetic nerve activity, contributing to its antihypertensive effects.
Dosage and Administration:
Adults: 5–10 mg once daily after breakfast. The dose may be increased up to 20 mg per day based on the patient's response and clinical condition.
Elderly: Initiate therapy with the lowest possible dose (5 mg) due to increased susceptibility to hypotension.
Pediatric Use: Safety and efficacy in patients under 18 years have not been established.
Administration: Take orally with water, preferably after meals. Do not crush or chew the tablet.
Interaction:
Antihypertensive agents: Concurrent use may enhance blood pressure-lowering effects.
CYP3A4 inhibitors: May increase plasma concentration of Cilnidipine.
Grapefruit juice: Avoid consumption as it can increase the absorption of Cilnidipine, leading to excessive hypotension.
Contraindications:
Known hypersensitivity to Cilnidipine or any component of the formulation.
Severe aortic stenosis.
Cardiogenic shock.
Side Effects:
Common: Headache, dizziness, flushing, palpitations, peripheral edema (especially ankle swelling), fatigue.
Rare: Gingival hyperplasia, skin rash, gastrointestinal discomfort.
Pregnancy & Lactation:
Pregnancy: Not recommended due to potential risk to the fetus. Use only if clearly needed and prescribed by a physician.
Lactation: Caution is advised; consult a physician before use.
Precautions & Warnings:
Use with caution in patients with severe hepatic or renal impairment.
Monitor blood pressure regularly to avoid excessive hypotension.
Abrupt discontinuation may lead to rebound hypertension; taper off under medical supervision.
