Indications:
Indicated for the treatment of chronic heart failure with reduced ejection fraction (HFrEF) and for symptomatic treatment of stable angina pectoris in patients with normal sinus rhythm.
Pharmacology:
Ivabradine selectively inhibits the funny current (If) in the sinoatrial node, which results in heart rate reduction without affecting myocardial contractility or intracardiac conduction.
Dosage:
Initially 2.5 mg twice daily. Dose may be increased based on heart rate and clinical response, under physician supervision.
Administration:
Should be taken with food, preferably at the same time every day. Avoid grapefruit juice.
Interaction:
May interact with diltiazem, verapamil, macrolide antibiotics, antifungals, and CYP3A4 inhibitors. Avoid combination with strong bradycardic agents.
Contraindications:
Hypersensitivity to Ivabradine, resting heart rate below 60 bpm, acute heart failure, severe hypotension, and pacemaker dependence.
Side Effects:
Bradycardia, visual disturbances (phosphenes), dizziness, fatigue, and atrial fibrillation may occur.
Pregnancy & Lactation:
Not recommended during pregnancy. Use during breastfeeding should be avoided due to potential adverse effects on infants.
Precautions & Warnings:
Regular monitoring of heart rate is essential. Caution in patients with atrial fibrillation, conduction defects, and liver impairment.
