Indications:
Indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus who are not adequately controlled on Linagliptin or Metformin XR alone.
Pharmacology:
Linagliptin: A DPP-4 enzyme inhibitor that works by increasing incretin levels, enhancing insulin secretion and decreasing glucagon secretion.
Metformin XR (Extended Release): A biguanide that decreases hepatic glucose production and improves insulin sensitivity while allowing for slower, steadier absorption over time.
Dosage:
Usual adult dose is 1 tablet once daily with the evening meal. Dosage may vary depending on patient response and prior treatment regimen.
Administration:
Take orally once daily with food (preferably dinner). Do not crush or chew; swallow whole with water.
Interaction:
Linagliptin: Interaction possible with CYP3A4 inducers like rifampicin.
Metformin XR: Caution with alcohol, contrast media, and other nephrotoxic drugs due to lactic acidosis risk.
Contraindications:
Severe renal impairment (eGFR <30 mL/min/1.73 m²)
Known hypersensitivity to Linagliptin or Metformin
Acute or chronic metabolic acidosis
Side Effects:
Common: Diarrhea, flatulence, nausea, and headache
Serious: Lactic acidosis (rare), pancreatitis, hypersensitivity reactions
Pregnancy & Lactation:
Not recommended unless clearly needed. Caution advised during breastfeeding as safety data is limited.
Precautions & Warnings:
Assess renal function before initiation and periodically during treatment.
Discontinue temporarily prior to iodinated contrast imaging procedures.
Watch for symptoms of lactic acidosis and pancreatitis.
