Indications:
Indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. Can be used as monotherapy or in combination with other antidiabetic agents.
Pharmacology:
Linagliptin is a DPP-4 inhibitor that increases incretin levels, which inhibit glucagon release, increase insulin secretion, and decrease blood glucose levels. It has a long half-life and primarily undergoes biliary excretion.
Dosage:
The recommended dose is 5 mg once daily, with or without food. Dose adjustment is not necessary for renal or hepatic impairment.
Administration:
Should be taken orally at the same time every day, with or without food. Swallow the tablet whole with water.
Interaction:
May interact with drugs such as rifampicin, sulfonylureas, and insulin. Co-administration with strong CYP3A4 inducers may reduce efficacy.
Contraindications:
Hypersensitivity to Linagliptin or any of the excipients. Not for use in type 1 diabetes or for the treatment of diabetic ketoacidosis.
Side Effects:
Generally well tolerated. Possible side effects include nasopharyngitis, cough, joint pain, pancreatitis, and hypersensitivity reactions.
Pregnancy & Lactation:
Use in pregnancy only if clearly needed. Caution should be exercised when administered to breastfeeding women due to lack of sufficient data.
Precautions & Warnings:
Not recommended in patients with type 1 diabetes. Monitor for signs of pancreatitis and hypersensitivity. Use caution when combining with insulin or sulfonylureas to prevent hypoglycemia.
