Indications:
SK-Cef is indicated for the treatment of infections caused by susceptible organisms, including:
Upper respiratory tract infections: pharyngitis, sinusitis, otitis media, tonsillitis, laryngo-tracheo-bronchitis.
Lower respiratory tract infections: acute and chronic bronchitis, lobar and bronchopneumonia.
Urinary tract infections: cystitis, urethritis, pyelonephritis.
Skin and soft tissue infections: abscess, cellulitis, furunculosis, impetigo.
Osteomyelitis and septic arthritis.
Bacterial sepsis and peritonitis.
Pharmacology:
Cephradine is a semisynthetic broad-spectrum bactericidal antibiotic belonging to the first-generation cephalosporins. It is active against both gram-positive and gram-negative microorganisms. Cephradine inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis.
Dosage:
Adults:
Urinary tract infections: 500 mg four times daily or 1 g twice daily.
Respiratory tract infections: 250–500 mg four times daily or 500 mg–1 g twice daily.
Skin and soft tissue infections: 250–500 mg four times daily or 500 mg–1 g twice daily.
Maximum daily dosage: 4 g.
Children:
General infections: 25–50 mg/kg/day in divided doses every 6–12 hours.
Otitis media: 75–100 mg/kg/day in divided doses every 6–12 hours.
Administration:
For oral suspension: Reconstitute the powder by adding 70 ml of boiled and cooled water in two portions, shaking well after each addition. Shake the suspension well before each use.
Can be taken with or without food; however, food may delay absorption slightly.
Interaction:
Concurrent use with nephrotoxic drugs (e.g., aminoglycosides) may increase the risk of kidney damage.
Probenecid may decrease renal clearance of Cephradine, leading to higher serum concentrations.
Contraindications:
Patients with known hypersensitivity to cephalosporin antibiotics.
Side Effects:
Gastrointestinal disturbances: nausea, vomiting, diarrhea, abdominal pain.
Hypersensitivity reactions: skin rashes, urticaria, pruritus.
Rarely: eosinophilia, leukopenia, neutropenia, positive Coombs test, pseudomembranous colitis.
Pregnancy & Lactation:
Use during pregnancy only if clearly needed and prescribed by a physician.
Cephradine is excreted in breast milk; caution is advised when administered to nursing mothers.
Precautions & Warnings:
Use with caution in patients with a history of hypersensitivity to penicillins, due to potential cross-reactivity.
Adjust dosage in patients with impaired renal function.
Therapeutic Class:
First-generation Cephalosporins.
Storage Conditions:
Store the dry powder at room temperature, protected from light and moisture.
After reconstitution, store the suspension in a refrigerator (2°C–8°C) and use within 7 days.
