Indications:
Acute migraine attacks
Mild to moderate pain
Post-operative pain

Pharmacology:
Tufnil 200mg Tablet contains tolfenamic acid, a member of the fenamate group of NSAIDs. It exerts its effects by inhibiting the biosynthesis of prostaglandins through the inhibition of cyclooxygenase (COX) enzymes, COX-1 and COX-2. Additionally, tolfenamic acid presents inhibitory actions on prostaglandin receptors and inhibits leukotriene B4 chemotaxis of human polymorphonuclear leukocytes, leading to anti-inflammatory and analgesic effects.
Dosage & Administration:
Acute migraine attacks: 200 mg when the first symptoms appear; may be repeated once after 1-2 hours if necessary.
Mild to moderate pain: 100-200 mg three times daily.
Renal impairment: Dose adjustments may be needed; severe impairment may require avoidance.
Administration: Should be taken with food or milk to minimize gastrointestinal discomfort.
Interaction:
Increased absorption: Metoclopramide and magnesium hydroxide may increase the absorption of tolfenamic acid.
Decreased absorption: Aluminum hydroxide may decrease the absorption.
Increased risk of bleeding: Co-administration with anticoagulants or other NSAIDs may increase the risk.
Altered drug effects: May reduce the effectiveness of antihypertensive drugs and increase plasma concentrations of lithium and methotrexate.
Contraindications:
Active peptic ulceration
Severe renal or hepatic impairment
History of gastrointestinal bleeding or perforation
Hypersensitivity to tolfenamic acid or other NSAIDs
Third trimester of pregnancy
Side Effects:
Common: Diarrhea, nausea, epigastric pain, vomiting, dyspepsia.
Less common: Gastric ulceration, erythema, pruritus, urticaria.
Rare: Dysuria, tremor, euphoria, fatigue, pulmonary infiltration, hematuria.
Serious: Blood dyscrasias, hepatitis.
Pregnancy & Lactation:
Pregnancy: Should not be used in the third trimester due to risks of premature closure of the ductus arteriosus and prolonged labor.
Lactation: Excreted in breast milk; use with caution during breastfeeding.
Precautions & Warnings:
Use with caution in patients with a history of gastrointestinal ulceration, impaired liver or kidney function, asthma, or cardiovascular diseases.
Monitor for signs of gastrointestinal bleeding or renal dysfunction during prolonged use.
Avoid concurrent use with other NSAIDs or anticoagulants.
Not recommended for use in children; a pediatric dosage regimen has not been established.
