Indications:
Lumona 4mg is indicated for:

  • Prophylaxis and chronic treatment of asthma.

  • Acute prevention of Exercise-Induced Bronchoconstriction (EIB).

  • Relief of symptoms of Seasonal and Perennial Allergic Rhinitis.

Pharmacology:
Montelukast is a selective leukotriene receptor antagonist that inhibits the effects of cysteinyl leukotrienes in the airways. These leukotrienes are involved in the pathophysiology of asthma and allergic rhinitis, leading to airway edema, smooth muscle contraction, and inflammation. By blocking leukotriene receptors, Montelukast reduces bronchoconstriction and inflammation.

Dosage:

  • Adults and adolescents (15 years and older): 10 mg once daily.

  • Pediatric patients (6 to 14 years): 5 mg once daily.

  • Pediatric patients (2 to 5 years): 4 mg once daily.

  • Pediatric patients (6 months to 2 years): 4 mg oral granules once daily.

Administration:
Administer orally, with or without food. For oral granules, they can be taken directly in the mouth or mixed with a spoonful of cold or room temperature soft food.

Interaction:
Montelukast can be co-administered with other therapies used in the prophylaxis and chronic treatment of asthma. No significant drug interactions have been observed with theophylline, prednisone, prednisolone, oral contraceptives, terfenadine, digoxin, and warfarin. However, co-administration with phenobarbital may decrease Montelukast levels; clinical monitoring is advised.

Contraindications:
Hypersensitivity to Montelukast or any component of the formulation.

Side Effects:

  • Common: Headache, diarrhea, nausea, vomiting, upper respiratory infection.

  • Uncommon: Dizziness, drowsiness, rash, abdominal pain.

  • Rare: Angioedema, hepatic dysfunction, eosinophilic granulomatosis with polyangiitis.

Pregnancy & Lactation:
Montelukast is classified as pregnancy category B. Animal studies have not shown adverse effects, but adequate studies in pregnant women are lacking. It is excreted in breast milk; caution is advised when administering to nursing mothers.

Precautions & Warnings:

  • Not indicated for acute asthma attacks; appropriate rescue medication should be available.

  • Should not be abruptly substituted for inhaled or oral corticosteroids.

  • Patients with aspirin-sensitive asthma should avoid aspirin and NSAIDs.

Therapeutic Class:
Leukotriene receptor antagonists.
