Indications:
Prophylaxis and chronic treatment of asthma in patients aged 2 to 5 years.
Relief of symptoms of seasonal allergic rhinitis in patients aged 2 years and older.
Prevention of exercise-induced bronchoconstriction in patients aged 6 years and older.
Pharmacology:
Montelukast is a selective and orally active leukotriene receptor antagonist. It binds with high affinity and selectivity to the cysteinyl leukotriene receptor type 1 (CysLT1), inhibiting the effects of cysteinyl leukotrienes (LTC₄, LTD₄, LTE₄) in the airways. This action reduces inflammation, bronchoconstriction, and other symptoms associated with asthma and allergic rhinitis.
Dosage:
Asthma and Allergic Rhinitis: 4 mg once daily in the evening for children aged 2 to 5 years.
Exercise-Induced Bronchoconstriction: 4 mg at least 2 hours before exercise for children aged 6 years and older.
Administration:
Oral administration.
Can be taken with or without food.
Tablets should be swallowed whole; do not chew or crush.
Interaction:
May interact with phenobarbital, phenytoin, and rifampin, which can decrease montelukast plasma concentrations.
No clinically significant interactions with theophylline, prednisone, or other commonly used asthma medications.
Contraindications:
Hypersensitivity to montelukast or any component of the formulation.
Side Effects:
Common: Headache, abdominal pain, cough, fever, and diarrhea.
Uncommon: Rash, dizziness, and fatigue.
Rare: Neuropsychiatric events such as agitation, aggression, anxiety, depression, and sleep disturbances.
Pregnancy & Lactation:
Pregnancy: Category B. Use during pregnancy only if clearly needed and prescribed by a healthcare provider.
Lactation: Excreted in breast milk; use with caution during breastfeeding.
Precautions & Warnings:
Not intended for the relief of acute asthma attacks.
Should not be used as monotherapy for the prevention of exercise-induced bronchoconstriction.
Patients with known aspirin sensitivity should continue avoidance of aspirin or non-steroidal anti-inflammatory agents while taking montelukast.
Discontinue use if neuropsychiatric symptoms occur.
