Indications:
Used as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when dual therapy with Linagliptin and Metformin is appropriate.
Pharmacology:
Linagliptin: A dipeptidyl peptidase-4 (DPP-4) inhibitor that increases levels of incretin hormones, which help regulate glucose by increasing insulin release and decreasing glucagon levels.
Metformin: A biguanide that reduces hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity.
Dosage:
Typically, one tablet twice daily with meals. Dosage should be personalized based on current antidiabetic therapy, effectiveness, and tolerance.
Administration:
Orally administered with meals to reduce gastrointestinal side effects. Tablets should be swallowed whole with water.
Interaction:
Linagliptin: May interact with CYP3A4 inducers like rifampicin, and insulin or sulfonylureas increasing hypoglycemia risk.
Metformin: Use with caution when combined with alcohol, cimetidine, or iodinated contrast agents due to lactic acidosis risk.
Contraindications:
Known hypersensitivity to Linagliptin or Metformin
Severe renal impairment (eGFR <30 mL/min/1.73 m²)
Acute or chronic metabolic acidosis (e.g., lactic acidosis)
Side Effects:
Common: Gastrointestinal discomfort, nausea, vomiting, flatulence, and headache
Serious but rare: Lactic acidosis (Metformin), pancreatitis or allergic reactions (Linagliptin)
Pregnancy & Lactation:
Use only if clearly needed and prescribed. The safety during lactation is not established, so consult a physician before breastfeeding.
Precautions & Warnings:
Monitor renal function periodically.
Temporarily discontinue before radiologic procedures involving contrast dye.
Be aware of symptoms of pancreatitis or metabolic acidosis.
