Pharmacology:
Clobetasol Propionate: A high-potency corticosteroid that reduces inflammation by stabilizing leukocyte lysosomal membranes, inhibiting macrophage accumulation, and decreasing capillary permeability.
Salicylic Acid: A keratolytic agent that softens and removes the stratum corneum by increasing hydration, leading to desquamation. It also possesses mild antiseptic properties.
Dosage & Administration:
Adults: Apply a thin layer to the affected area twice daily and rub in gently.
Children: Not recommended for use in pediatric patients under 12 years of age.
Duration: Treatment should not exceed 2 consecutive weeks. If no improvement is seen within 2 weeks, reassessment of diagnosis may be necessary.
Occlusion: Should not be used with occlusive dressings unless directed by a healthcare provider.
Interaction:
Topical salicylic acid may increase the absorption of other topically applied medicines. Concomitant use with other topical medications on the same area should be avoided.
Contraindications:
Hypersensitivity to clobetasol propionate, salicylic acid, or any component of the ointment.
Primary infected bacterial or fungal skin lesions if no anti-infective agent is used simultaneously.
Primary cutaneous viral infections (e.g., herpes simplex, vaccinia, varicella).
Tuberculous skin lesions.
Dermatoses in children under 1 year of age, including dermatitis and diaper eruptions.
Side Effects:
Common: Burning, stinging, or itching at the application site.
Less Common: Skin thinning, striae, and dilatation of superficial blood vessels.
Rare: Allergic reactions, including rash or swelling.
Pregnancy & Lactation:
Pregnancy: Use only if the potential benefit justifies the potential risk to the fetus.
Lactation: It is not known whether clobetasol propionate is excreted in human milk. Caution should be exercised when administered to a nursing woman.
Precautions & Warnings:
Prolonged use may lead to systemic absorption and potential side effects.
Use with caution in patients with impaired peripheral circulation or diabetes.
Avoid application on broken skin, mucous membranes, or the anogenital region.
Not for prolonged use in high concentrations and on large areas of the body.
Therapeutic Class:
Corticosteroid and Keratolytic Combination
