Pharmacology:

Cephradine is a semisynthetic broad-spectrum bactericidal antibiotic belonging to the first-generation cephalosporins. It inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis.

Dosage:

    • Maximum daily dose: 4 g.

  • Children:

    • General infections: 25–50 mg/kg/day in divided doses every 6–12 hours.

    • Otitis media: 75–100 mg/kg/day in divided doses every 6–12 hours.

Administration:

  • Take with or without food.

  • Swallow the capsule whole with a glass of water.

Interaction:

  • Concurrent use with nephrotoxic drugs (e.g., aminoglycosides) may increase the risk of kidney damage.

  • Probenecid may decrease renal clearance of Cephradine, leading to higher serum concentrations.

Contraindications:

  • Patients with known hypersensitivity to cephalosporin antibiotics.

Side Effects:

  • Gastrointestinal disturbances: nausea, vomiting, diarrhea, abdominal pain.

  • Hypersensitivity reactions: skin rashes, urticaria, pruritus.

  • Rarely: eosinophilia, leukopenia, neutropenia, positive Coombs test, pseudomembranous colitis.

Pregnancy & Lactation:

  • Use during pregnancy only if clearly needed and prescribed by a physician.

  • Cephradine is excreted in breast milk; caution is advised when administered to nursing mothers.

Precautions & Warnings:

  • Use with caution in patients with a history of hypersensitivity to penicillins, due to potential cross-reactivity.

  • Adjust dosage in patients with impaired renal function.

First-generation Cephalosporins.
