Indications:
Indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with both Linagliptin and Metformin is appropriate.
Pharmacology:
This combination includes:
Linagliptin: A DPP-4 inhibitor that enhances the body's own ability to lower blood glucose by increasing incretin levels.
Metformin: A biguanide that decreases hepatic glucose production and improves insulin sensitivity in peripheral tissues.
Dosage:
The usual dose is 1 tablet twice daily with meals. Dosage should be individualized based on patient’s current treatment, effectiveness, and tolerability.
Administration:
Oral administration with food to minimize gastrointestinal side effects. Swallow whole with water.
Interaction:
Linagliptin: May interact with rifampicin, insulin, and sulfonylureas.
Metformin: Interacts with alcohol, cimetidine, and iodine-containing contrast agents, increasing the risk of lactic acidosis.
Contraindications:
Renal impairment (eGFR <30 mL/min/1.73 m²)
Metabolic acidosis or diabetic ketoacidosis
Hypersensitivity to any component
Side Effects:
Common: Nausea, diarrhea, flatulence, and headache
Rare: Lactic acidosis (Metformin), pancreatitis (Linagliptin), allergic reactions
Pregnancy & Lactation:
Should be used during pregnancy only if clearly needed. Avoid breastfeeding or consult a physician before use due to unknown excretion in breast milk.
Precautions & Warnings:
Monitor renal function regularly.
Stop Metformin temporarily before any radiologic study with iodinated contrast.
Be alert for signs of pancreatitis or lactic acidosis.
