Indications:
Indicated for the management of hypertension and chronic stable angina. Also used in vasospastic angina and other cardiovascular conditions requiring blood pressure control.
Pharmacology:
Amlodipine is a long-acting dihydropyridine calcium channel blocker. It inhibits the influx of calcium ions into vascular smooth muscle and cardiac muscle, leading to vasodilation and reduced blood pressure. It also improves oxygen delivery to the heart in angina.
Dosage:
Adults: 5–10 mg once daily, depending on response and condition.
Elderly or hepatic impairment: Start with lower dose (2.5–5 mg/day).
Administration:
To be taken orally once daily, with or without food. Swallow whole with water.
Interaction:
May interact with other antihypertensives, simvastatin, CYP3A4 inhibitors (e.g., ketoconazole), and grapefruit juice (may increase blood levels of Amlodipine).
Contraindications:
Contraindicated in patients with known hypersensitivity to Amlodipine or other dihydropyridines. Also contraindicated in severe hypotension, cardiogenic shock, and aortic stenosis.
Side Effects:
Common: Swelling of ankles or feet (peripheral edema), dizziness, flushing, headache, and fatigue.
Rare: Palpitations, nausea, abdominal pain, and allergic reactions.
Pregnancy & Lactation:
Should be used during pregnancy only if clearly needed. Use in lactation should be decided by the physician based on risk-benefit analysis.
Precautions & Warnings:
Use cautiously in patients with heart failure, liver impairment, or low blood pressure. Sudden withdrawal is not recommended.
