Indications:
Treatment of active duodenal and gastric ulcers
Gastroesophageal reflux disease (GERD)
Erosive esophagitis
Zollinger-Ellison syndrome
Prevention of ulcer recurrence and stress ulcers
Pharmacology:
Famotidine is a competitive histamine H2 receptor antagonist that acts on the parietal cells of the stomach to inhibit both basal and stimulated gastric acid secretion. It effectively lowers the volume and concentration of stomach acid, helping heal ulcers and prevent acid reflux.
Dosage:
Duodenal ulcer: 40 mg once daily at bedtime for 4–8 weeks
Gastric ulcer: 40 mg once daily at bedtime
GERD and esophagitis: 20–40 mg twice daily for 6–12 weeks
Zollinger-Ellison syndrome: Start with 20 mg every 6 hours, titrate as needed
Dose adjustment is needed for patients with renal impairment
Administration:
Administer orally with or without food
Best taken at bedtime or as directed
Swallow the tablet whole with water
Interaction:
May decrease absorption of drugs needing acidic pH (e.g., ketoconazole, atazanavir)
Antacids may be used concomitantly
No major CYP450 enzyme interactions
Contraindications:
Known hypersensitivity to Famotidine or other H2 blockers
Use with caution in severe renal impairment
Side Effects:
Common: Headache, diarrhea, dizziness, constipation
Rare: Confusion (especially in elderly), hallucinations, bradycardia, hepatitis
Prolonged use may result in vitamin B12 deficiency
Pregnancy & Lactation:
Category B: No proven risk in humans
Use cautiously during breastfeeding
Precautions & Warnings:
Rule out gastric malignancy before treatment
Caution in elderly and renal-impaired patients
Avoid abrupt discontinuation in chronic users
