Indications:
Cardizem 60 mg is indicated for the treatment of:
Chronic stable angina
Hypertension (high blood pressure)
Supraventricular arrhythmias including atrial fibrillation and atrial flutter
Pharmacology:
Diltiazem is a nondihydropyridine calcium channel blocker that inhibits calcium ion influx across cardiac and vascular smooth muscle, resulting in vasodilation and decreased myocardial contractility. This reduces blood pressure and myocardial oxygen demand, and improves oxygen supply.
Dosage:
Adult dose: 60 mg 3 to 4 times daily, typically before meals and at bedtime.
Dosage should be individualized based on patient response.
Administration:
Take orally, preferably before meals. Swallow the tablet whole with water. Do not crush or chew.
Interaction:
May interact with:
Beta-blockers (e.g., atenolol, metoprolol)
Digoxin
CYP3A4 inhibitors and inducers
Statins (increased risk of myopathy)
Contraindications:
Sick sinus syndrome (without pacemaker)
Second or third-degree AV block
Severe hypotension
Acute myocardial infarction with pulmonary congestion
Hypersensitivity to Diltiazem
Side Effects:
Common: Dizziness, headache, flushing, fatigue, edema, bradycardia
Less common: Nausea, constipation, rash, AV block, liver enzyme elevation
Pregnancy & Lactation:
Use only if clearly needed during pregnancy.
Diltiazem is excreted in human milk; breastfeeding is not recommended unless benefits outweigh risks.
Precautions & Warnings:
Monitor blood pressure and heart rate regularly.
Use cautiously in patients with hepatic or renal impairment.
May worsen heart failure or conduction abnormalities.
