Indications:
D Toin 100mg is indicated for the control of generalized tonic-clonic (grand mal) and complex partial seizures. It is also used for prevention and treatment of seizures during or following neurosurgery.
Pharmacology:
Phenytoin Sodium stabilizes neuronal membranes and decreases seizure activity by increasing efflux or decreasing influx of sodium ions across cell membranes in the motor cortex during generation of nerve impulses.
Dosage:
Adults: Typically 100 mg three times daily. Dosage may be adjusted based on patient response and plasma level monitoring.
Children: Dose depends on weight and clinical response.
Administration:
Should be taken orally with food to minimize gastrointestinal discomfort. Do not crush or chew the capsule; swallow whole with water.
Interaction:
Phenytoin interacts with many drugs including warfarin, oral contraceptives, and other antiepileptic drugs. Enzyme-inducing properties may alter plasma levels of co-administered drugs.
Contraindications:
Contraindicated in patients with hypersensitivity to Phenytoin or other hydantoins, and in those with sinus bradycardia, sino-atrial block, or second and third-degree AV block.
Side Effects:
Common side effects include dizziness, drowsiness, unsteady movement, and nausea. Long-term use may cause gum overgrowth, hirsutism, or bone softening. Rarely, severe allergic reactions may occur.
Pregnancy & Lactation:
Use with caution during pregnancy (Category D). May cause fetal harm. Monitor newborns for withdrawal symptoms. Phenytoin is excreted in breast milk.
Precautions & Warnings:
Regular monitoring of blood levels is recommended. Use with caution in patients with liver disease, diabetes, or porphyria. Do not abruptly discontinue. Maintain good oral hygiene.
