Indications:

NOMOSIC 50 MG is primarily indicated for:

Erectile Dysfunction (ED): To help men who are unable to achieve or maintain an erection sufficient for satisfactory sexual performance.
Pulmonary Arterial Hypertension (PAH): Sometimes used off-label to treat pulmonary hypertension by relaxing the blood vessels in the lungs and improving blood flow (note: this is less common compared to ED treatment).

Pharmacology:

NOMOSIC contains Sildenafil, which belongs to a class of drugs known as phosphodiesterase type 5 inhibitors (PDE5 inhibitors). Sildenafil works by inhibiting the PDE5 enzyme, which normally breaks down cyclic guanosine monophosphate (cGMP) in the smooth muscle cells of blood vessels. cGMP is responsible for relaxing the smooth muscles and dilating the blood vessels.

In men with erectile dysfunction, Sildenafil promotes increased blood flow to the penis during sexual stimulation, helping to achieve and maintain an erection. Sildenafil has no effect without sexual stimulation, as it does not cause spontaneous erections.

Dosage:

For Erectile Dysfunction (ED): The usual recommended dose of NOMOSIC 50 MG is 50 mg, taken approximately 30 minutes to 1 hour before sexual activity. The dose can be adjusted based on individual effectiveness and tolerance, with the maximum recommended dose being 100 mg and the minimum dose being 25 mg.
For Pulmonary Arterial Hypertension (PAH): This is an off-label use, and dosing would be determined by a healthcare professional. Generally, for PAH, Sildenafil is prescribed at a lower dose than for ED.

Note: Only one dose per day is recommended. Do not exceed the prescribed dose.

Administration:

NOMOSIC 50 MG should be taken orally, with or without food.
Swallow the tablet whole with a glass of water.
Do not chew or crush the tablet.
For best results, take the medication about 30 minutes to 1 hour before engaging in sexual activity.
Do not take more than one dose within 24 hours.

Interaction:

NOMOSIC 50 MG may interact with:

Nitrates (e.g., nitroglycerin): Combining Sildenafil with nitrates can lead to a severe drop in blood pressure, which can be dangerous. This combination should be avoided.
Alpha-blockers (e.g., doxazosin): Taking Sildenafil with alpha-blockers can also cause a significant decrease in blood pressure.
Antifungal medications (e.g., ketoconazole), HIV protease inhibitors (e.g., ritonavir), and macrolide antibiotics (e.g., erythromycin): These drugs can increase the blood levels of Sildenafil, potentially enhancing side effects.
Grapefruit juice: Grapefruit juice can increase the concentration of Sildenafil in the blood, leading to increased risk of side effects.

Always inform your healthcare provider about any other medications you are currently taking.

Contraindications:

Hypersensitivity to Sildenafil or any other ingredients in the formulation.
Nitrate medication use: Patients who are using nitrates (for chest pain or heart problems) should not take Sildenafil, as this can lead to a dangerous drop in blood pressure.
Severe heart or liver problems: Individuals with severe heart conditions, especially those who have had a recent heart attack or stroke, should avoid taking Sildenafil.
Severe hypotension (low blood pressure) or uncontrolled high blood pressure.
Retinitis pigmentosa (a rare genetic eye disease), as Sildenafil may increase the risk of vision problems in such patients.
Priapism history: Men who have had an erection lasting more than 4 hours in the past should avoid using this drug.

Side Effects:

Common side effects: Headache, flushing, nasal congestion, dizziness, indigestion, and visual disturbances (e.g., blurred vision, color tinge).
Serious side effects: Prolonged erection (priapism), sudden loss of vision, sudden hearing loss, chest pain, shortness of breath, and irregular heartbeat.
Other side effects: Rashes, back pain, muscle aches, and nausea.

Pregnancy & Lactation:

Pregnancy: Sildenafil is not recommended for use during pregnancy, as its safety in pregnant women has not been established.
Lactation: Sildenafil is not recommended for use during breastfeeding, as it is not known if it is excreted in breast milk. Consult your doctor before using this medication while breastfeeding.
