Indications:
Nidocard Retard 2.6 mg Tablet is indicated for the prophylaxis of chronic stable angina pectoris due to coronary artery disease. It is not suitable for treating an acute anginal attack as the onset of action is not sufficiently rapid .
Pharmacology:
Nidocard Retard 2.6 mg Tablet contains nitroglycerin, which is converted to nitric oxide in the body. Nitric oxide activates guanylate cyclase, leading to increased levels of cyclic GMP. This results in the relaxation of vascular smooth muscles, causing vasodilation of both peripheral arteries and veins. The dilation of veins reduces venous return to the heart (preload), and the dilation of arteries reduces systemic vascular resistance (afterload), thereby decreasing the heart's workload and relieving anginal symptoms .
Dosage & Administration:
For adults, one 2.6 mg sustained-release tablet should be taken in the morning and evening on an empty stomach. The dosage should be adjusted according to the patient's response and the severity of the condition .
Administration:
Administer orally with a full glass of water. Do not chew or crush the tablet. Take on an empty stomach for optimal absorption.
Interaction:
Concomitant use of nitrates with antihypertensive drugs, beta-adrenergic blockers, or calcium channel blockers may enhance the hypotensive effects. Concurrent use with phosphodiesterase type 5 inhibitors (e.g., sildenafil) can cause a significant drop in blood pressure. Alcohol may also potentiate the hypotensive effects of nitrates .
Contraindications:
Nidocard Retard 2.6 mg Tablet is contraindicated in patients with known hypersensitivity to nitroglycerin or other nitrates, severe anemia, closed-angle glaucoma, head trauma, cerebral hemorrhage, or increased intracranial pressure. It should not be used concomitantly with phosphodiesterase type 5 inhibitors .
Side Effects:
Common side effects include headache, dizziness, lightheadedness, nausea, and flushing. Less common but serious side effects may include hypotension, tachycardia, and syncope. If any severe side effects occur, seek medical attention promptly .
Pregnancy & Lactation:
Nidocard Retard 2.6 mg Tablet should be used during pregnancy and lactation only if clearly needed and as directed by a healthcare provider. The safety profile in these populations has not been well established .
