Indications:
Dialiptin M 500mg is prescribed for the management of type 2 diabetes mellitus, especially in patients who do not achieve adequate glycemic control with diet and exercise alone or with monotherapy.
Pharmacology:
Metformin: A biguanide that reduces hepatic glucose production and increases insulin sensitivity, improving peripheral glucose uptake.
Sitagliptin: A DPP-4 inhibitor that enhances incretin hormone levels, leading to increased insulin release and decreased glucagon secretion, thereby lowering blood glucose.
Dosage:
Usually, one tablet once or twice daily with meals, as prescribed by the physician.
Dosage may be adjusted based on blood glucose monitoring and patient response.
Administration:
Oral administration with meals to reduce gastrointestinal side effects. Swallow whole with water.
Interaction:
May interact with cationic drugs eliminated by renal tubular secretion (e.g., digoxin), other antidiabetics, and diuretics. Caution advised with iodinated contrast agents.
Contraindications:
Known hypersensitivity to Metformin or Sitagliptin
Severe renal impairment or acute conditions causing hypoxia
Metabolic acidosis including diabetic ketoacidosis
Severe liver disease
Side Effects:
Common side effects include nausea, vomiting, diarrhea, abdominal discomfort, headache, and rare hypoglycemia. Lactic acidosis is a rare but serious side effect of Metformin.
Pregnancy & Lactation:
Use only if clearly needed and prescribed by a physician. Limited data available; Metformin crosses placenta and is excreted in breast milk.
Precautions & Warnings:
Monitor renal function regularly
Caution in elderly patients and those with renal or hepatic impairment
Avoid excessive alcohol intake
Educate patient on symptoms of hypoglycemia and lactic acidosis
