Indications:
D-Cap 20,000 IU is indicated for:

  • Treatment and prevention of vitamin D3 deficiency.

  • Adjunct to specific therapy for osteoporosis, osteomalacia, hypocalcemia, tetany, and rickets in patients with vitamin D3 deficiency.

Pharmacology:
Cholecalciferol (Vitamin D3) is a fat-soluble vitamin that, after absorption in the small intestine, is converted in the liver to 25-hydroxyvitamin D3 (calcidiol), and then in the kidneys to the active form, 1,25-dihydroxyvitamin D3 (calcitriol). Calcitriol binds to vitamin D receptors in various tissues, promoting calcium and phosphate absorption, and maintaining bone health.

Dosage:

  • Adults:

    • Treatment of Vitamin D3 deficiency: 40,000 IU once weekly for 7 weeks.

    • Maintenance therapy: 1,400–2,000 IU daily.

    • Prevention of deficiency: 20,000 IU every 4 weeks.

    • Adjunct to osteoporosis therapy: 20,000 IU once a month.

  • Children (12–18 years):

    • Treatment of Vitamin D3 deficiency: 20,000 IU once every 2 weeks for 6 weeks.

    • Prevention of deficiency: 20,000 IU every 6 weeks.

Administration:
Take orally with or within 1 hour after a meal. Swallow the capsule whole with water.

Interaction:
Cholecalciferol may interact with:

  • Anticonvulsants: Such as phenytoin and phenobarbital, which can reduce vitamin D effectiveness.

  • Glucocorticoids: May impair vitamin D metabolism.

  • Thiazide diuretics: Can increase the risk of hypercalcemia when taken with vitamin D.

Contraindications:

  • Hypersensitivity to cholecalciferol or any component of the formulation.

  • Hypercalcemia or hypervitaminosis D.

  • Severe renal impairment.

Side Effects:
Generally well-tolerated. Possible side effects include:

  • Nausea

  • Vomiting

  • Constipation

  • Weakness

  • Fatigue

Excessive doses may lead to hypercalcemia, presenting with symptoms like confusion, arrhythmias, and kidney dysfunction.

Pregnancy & Lactation:
Vitamin D3 is considered safe during pregnancy and lactation when used within recommended doses. However, consult a physician before use.
