Indications:
Linarol-M 850 mg is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with both Linagliptin and Metformin is appropriate.

Pharmacology:

  • Linagliptin: A DPP-4 inhibitor that increases and prolongs the activity of incretin hormones, leading to enhanced insulin release and reduced glucagon secretion in a glucose-dependent manner.

  • Metformin Hydrochloride: A biguanide that decreases hepatic glucose production, decreases intestinal glucose absorption, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization.

Dosage:

  • Initial Dose: In patients not currently treated with Metformin, initiate with 2.5 mg Linagliptin and 500 mg Metformin twice daily with meals.

  • Maintenance Dose: The dosage should be individualized based on effectiveness and tolerability, not exceeding 2.5 mg Linagliptin and 1000 mg Metformin twice daily with meals.

Administration:
Take the tablet orally with meals to reduce gastrointestinal side effects associated with Metformin.

Interaction:

  • Cationic Drugs: May reduce Metformin elimination.

  • P-glycoprotein/CYP3A4 Inducers: Rifampin may decrease Linagliptin exposure, reducing its efficacy.

  • Other Drugs Affecting Glycemic Control: Certain drugs may produce hyperglycemia, leading to loss of glycemic control.

Contraindications:

  • Hypersensitivity to Linagliptin or Metformin Hydrochloride.

  • Renal impairment (eGFR <30 mL/min/1.73 m²).

  • Acute or chronic metabolic acidosis, including diabetic ketoacidosis.

  • Hepatic insufficiency.

Side Effects:

  • Common: Nasopharyngitis, diarrhea.

  • Less Common: Hypoglycemia (especially when used with sulfonylureas), nausea, vomiting, abdominal discomfort.

Pregnancy & Lactation:

  • Pregnancy: Category B. Should be used during pregnancy only if clearly needed.

  • Lactation: Caution should be exercised when administered to a nursing woman.
