Indications:

NIDOCARD 400 MCG is primarily indicated for the treatment and prevention of:

  • Angina (Chest Pain): NIDOCARD is used to relieve chest pain caused by reduced blood flow to the heart, often associated with coronary artery disease.

  • Acute Coronary Syndrome: To relieve chest pain and reduce heart workload in the case of a heart attack or unstable angina.

  • Hypertension: Occasionally used for lowering blood pressure, though it is not the first-line treatment.

  • Heart Failure: In some cases, it is used in heart failure to improve symptoms by reducing the heart's workload.

Pharmacology:

NIDOCARD contains Nitroglycerin, a nitrate that works by relaxing the smooth muscles in the walls of blood vessels. This causes the blood vessels to dilate (widen), improving blood flow to the heart and reducing the heart's workload. Nitroglycerin primarily acts on the veins, which reduces the amount of blood returning to the heart, thereby decreasing the heart's demand for oxygen. It also helps relieve the pain associated with angina by increasing the oxygen supply to the heart muscle.

Dosage:

  • For Angina (Chest Pain): The typical starting dose is 400 mcg once or twice daily. The dose may be increased depending on the patient's response to the medication.

  • For Prevention: It may be prescribed to prevent episodes of angina, especially during activities that may trigger chest pain, like exercise or stress.

  • Maximum Dose: The daily dose may vary depending on the physician's assessment of the condition but should not exceed the recommended dose.

Administration:

  • NIDOCARD 400 MCG should be taken sublingually (under the tongue) or as directed by your doctor.

  • It should be taken while the mouth is dry and the tablet should be allowed to dissolve completely.

  • Do not swallow or chew the tablet.

  • Follow the prescribed dosage and timing carefully. Do not take extra doses unless instructed by your doctor.

Interaction:

NIDOCARD 400 MCG may interact with:

  • Other vasodilators (e.g., sildenafil, tadalafil, vardenafil): May cause excessive lowering of blood pressure, leading to dizziness or fainting.

  • Antihypertensive drugs: May amplify the blood pressure-lowering effect of Nitroglycerin.

  • Beta-blockers and Calcium Channel Blockers: When combined with nitrates, these medications may lead to an excessive reduction in heart rate or blood pressure.

  • Alcohol: Concurrent use of alcohol may increase the risk of dizziness and fainting due to the blood pressure-lowering effects of Nitroglycerin.

  • Always inform your healthcare provider of any other medications you are taking, including over-the-counter drugs and supplements.

Contraindications:

  • Hypersensitivity to Nitroglycerin or any of the components of the formulation.

  • Severe hypotension (low blood pressure) or shock.

  • Severe anemia: As Nitroglycerin may further reduce oxygen delivery to tissues.

  • Increased intracranial pressure: Nitroglycerin should not be used in individuals with head trauma or increased intracranial pressure.

  • Concurrent use of PDE5 inhibitors (e.g., sildenafil) for erectile dysfunction, as this can lead to a dangerous drop in blood pressure.

Side Effects:

  • Common side effects: Headache, dizziness, lightheadedness, flushing, and nausea. These are common when starting the medication, as the blood vessels dilate.

  • Serious side effects: Severe hypotension (low blood pressure), fainting, and excessive dizziness.

  • Other side effects: Tachycardia (fast heart rate), palpitations, and allergic reactions (e.g., rash, itching, swelling).

Pregnancy & Lactation:

  • Pregnancy: Nitroglycerin should only be used during pregnancy if the potential benefit justifies the potential risk to the fetus. It is classified as a Category C drug during pregnancy.

  • Lactation: It is not known if Nitroglycerin passes into breast milk. Consult your doctor before using this medication if breastfeeding.

Therapeutic Class:

Vasodilator; Nitrate medication
