Indications:
Prostam 0.4 mg Capsule is indicated for:

  • Relief of lower urinary tract symptoms associated with benign prostatic hyperplasia (BPH).

  • Adjunct treatment of acute urinary retention (AUR) and bladder outlet obstruction (BOO).

  • Expulsion of ureteral stones up to 5 mm in size.

Pharmacology:
Tamsulosin hydrochloride is a selective alpha-1 adrenergic blocker that predominantly targets the alpha-1A receptors in the prostate. By blocking these receptors, tamsulosin causes relaxation of smooth muscle in the prostate and bladder neck, leading to improved urine flow and relief from urinary symptoms associated with BPH.

Dosage:

  • Initial Dose: 0.4 mg orally once daily.

  • Maximum Dose: 0.8 mg orally once daily.

  • Administer approximately 30 minutes after the same meal each day to enhance absorption.

Administration:
Swallow the capsule whole with water. Do not chew or crush. For patients who have difficulty swallowing capsules, consult a healthcare provider for alternative administration methods.

Interaction:

  • Increased plasma concentration: Concurrent use with strong CYP3A4 inhibitors (e.g., ketoconazole), moderate CYP3A4 inhibitors (e.g., erythromycin), and cimetidine may increase tamsulosin levels.

  • Additive hypotensive effect: Combining with other alpha-1 blockers or antihypertensive agents may enhance blood pressure-lowering effects.

  • Decreased plasma concentration: Concurrent use with furosemide may reduce tamsulosin levels.

Contraindications:

  • Known hypersensitivity to tamsulosin or any component of the formulation.

  • History of orthostatic hypotension.

  • Severe hepatic impairment (Child-Pugh scores > 9).

  • Severe renal impairment with creatinine clearance <10 mL/min.

  • Concurrent use with other alpha-1 adrenergic blockers.

Side Effects:
Common side effects include:

  • Dizziness

  • Abnormal ejaculation (e.g., retrograde ejaculation)

  • Headache

  • Asthenia (weakness)

  • Postural hypotension

  • Palpitations

  • Rhinitis

Less common side effects may include gastrointestinal disturbances such as nausea, vomiting, diarrhea, and constipation.

Pregnancy & Lactation:
Prostam 0.4 mg Capsule is not recommended for use in women and children. Safety during pregnancy and lactation has not been established. Consult a healthcare provider before use during these periods.
