Indications:
Indicated for the treatment and prophylaxis of severe ventricular arrhythmias, including ventricular tachycardia and ventricular fibrillation, especially when unresponsive to other therapies. Also used in supraventricular arrhythmias including atrial fibrillation.

Pharmacology:
Amiodarone is a class III antiarrhythmic that prolongs the action potential duration and refractory period in all cardiac tissues. It inhibits adrenergic stimulation and blocks potassium channels, along with sodium and calcium channels, helping to stabilize cardiac rhythm.

Dosage:
Initial: 800–1600 mg/day in divided doses for 1–3 weeks, then reduce to 600–800 mg/day for 1 month.
Maintenance: 200–400 mg/day, based on response.

Administration:
To be taken orally with meals to enhance absorption and reduce gastrointestinal side effects. Swallow whole with water.

Interaction:
May interact with digoxin, warfarin, beta-blockers, calcium channel blockers, and other antiarrhythmic agents. Grapefruit juice can increase blood levels.

Contraindications:
Contraindicated in patients with sinus bradycardia, AV block (unless with pacemaker), iodine hypersensitivity, thyroid dysfunction, and severe respiratory or liver disease.

Side Effects:
Common: Nausea, vomiting, fatigue, tremors, photosensitivity, blue-grey skin discoloration.
Serious: Pulmonary toxicity, liver dysfunction, thyroid imbalance, and corneal deposits.

Pregnancy & Lactation:
Contraindicated during pregnancy and breastfeeding due to risk of fetal toxicity and accumulation in breast milk.

Precautions & Warnings:
Regular monitoring of liver, thyroid, lungs, and eyes is essential. Avoid prolonged sun exposure. Use with caution in elderly and those with hepatic or pulmonary issues.
