Indications:
Used for the prevention of angina pectoris due to coronary artery disease. Not intended for relief of acute anginal attacks.
Pharmacology:
Isosorbide Mononitrate is a long-acting nitrate. It relaxes vascular smooth muscle, dilating peripheral arteries and veins, which reduces preload and afterload on the heart. This lowers myocardial oxygen demand and helps prevent anginal episodes.
Dosage:
Typical dose: 30–60 mg once daily in the morning. Dosage may be adjusted based on response.
Extended-release forms should not be crushed or chewed.
Administration:
Administer orally with or without food. Swallow the tablet whole with water.
Interaction:
May interact with PDE5 inhibitors (e.g., sildenafil), antihypertensives, and alcohol, potentially causing severe hypotension.
Contraindications:
Contraindicated in patients with known hypersensitivity to nitrates, acute myocardial infarction with low filling pressure, severe anemia, and concurrent use with PDE5 inhibitors.
Side Effects:
Common: Headache, dizziness, lightheadedness, flushing.
Rare: Hypotension, tachycardia, nausea.
Pregnancy & Lactation:
Use only if clearly needed during pregnancy. Caution advised during lactation due to limited data.
Precautions & Warnings:
Tolerance can develop with continuous use; a nitrate-free interval is recommended. Use cautiously in patients with hypotension, hypovolemia, or glaucoma.
