Indications:
Gastric and duodenal ulcers
Gastroesophageal reflux disease (GERD)
Zollinger-Ellison syndrome
Heartburn and acid indigestion
Prevention of ulcer recurrence
Pharmacology:
Famotidine is a competitive histamine H2 receptor antagonist. It inhibits basal and stimulated gastric acid secretion by blocking H2 receptors on the parietal cells in the stomach lining. It reduces both the volume and acidity of gastric secretions.
Dosage:
For ulcers: 20 mg twice daily or 40 mg once at bedtime
GERD: 20 mg twice daily for up to 6 weeks
Zollinger-Ellison syndrome: Initial dose 20 mg every 6 hours; may be adjusted
Dose should be adjusted in patients with renal impairment
Administration:
Oral administration, with or without food
Swallow whole with water
Best taken at bedtime for ulcer healing
Interaction:
May affect absorption of drugs dependent on gastric pH (e.g., ketoconazole, itraconazole)
No significant interactions with antacids
May alter the efficacy of atazanavir and certain antifungal medications
Contraindications:
Hypersensitivity to Famotidine or other H2-receptor antagonists
Caution in patients with renal dysfunction
Side Effects:
Common: Headache, constipation, diarrhea, dizziness
Rare: Bradycardia, confusion (especially in elderly), hepatitis, arrhythmias
Long-term use may lead to vitamin B12 deficiency
Pregnancy & Lactation:
Pregnancy category B: No proven risk in humans
Use with caution during breastfeeding as it may be excreted in breast milk
Precautions & Warnings:
Evaluate for malignancy in gastric ulcer cases before starting therapy
Monitor renal function in long-term use
Use cautiously in elderly patients
Avoid abrupt discontinuation after long-term therapy
