Indications:
M-Kast 5 mg is indicated for:

  • Prophylaxis and chronic treatment of asthma in patients 12 months of age and older.

  • Acute prevention of Exercise-Induced Bronchoconstriction (EIB) in patients 6 years of age and older.

  • Relief of symptoms of Allergic Rhinitis (AR): Seasonal Allergic Rhinitis (SAR) in patients 2 years of age and older, and perennial allergic rhinitis (PAR) in patients 6 months of age and older.

Pharmacology:
Montelukast is a selective and orally active leukotriene receptor antagonist. It works by inhibiting the cysteinyl leukotriene receptor (CysLT₁), thereby preventing the physiological effects of leukotrienes, which are involved in the inflammatory process of asthma and allergic rhinitis.

Dosage:

  • Asthma and Allergic Rhinitis:

    • Adults and adolescents (15 years and older): 10 mg once daily in the evening.

    • Pediatric patients (6 to 14 years): 5 mg once daily in the evening.

    • Pediatric patients (2 to 5 years): 4 mg once daily in the evening.

    • Pediatric patients (6 months to 5 years): 4 mg oral granules once daily in the evening.

  • Exercise-Induced Bronchoconstriction (EIB):

    • Adults and adolescents (15 years and older): 10 mg at least 2 hours before exercise; do not take additional doses within 24 hours.

    • Pediatric patients (6 to 14 years): 5 mg at least 2 hours before exercise; do not take additional doses within 24 hours.

Administration:
Take the tablet orally with or without food, swallowed whole with a glass of water. For pediatric patients, the tablet may be chewed or taken with a spoonful of cold water or soft food at room temperature.

Interaction:
M-Kast has been administered with other therapies routinely used in the prophylaxis and chronic treatment of asthma with no apparent increase in adverse reactions. In drug interaction studies, the recommended clinical dose of M-Kast did not have clinically important effects on the pharmacokinetics of the following drugs: theophylline, prednisone, prednisolone, oral contraceptives (norethindrone 1mg/ethinyl estradiol 35mcg), terfenadine, digoxin, and warfarin. However, caution should be exercised when coadministered with potent cytochrome P450 enzyme inducers, such as phenobarbital or rifampicin.

Contraindications:
Montelukast is contraindicated in patients who are hypersensitive to any component of this product.

Side Effects:
Common side effects include:

  • Diarrhea

  • Fever

  • Gastrointestinal discomfort

  • Headache

  • Nausea

  • Vomiting

  • Skin reactions

  • Upper respiratory tract infection

Less common side effects may include:

  • Akathisia

  • Anxiety

  • Arthralgia

  • Asthenia

  • Abnormal behavior

  • Depression

  • Dizziness

  • Drowsiness

  • Dry mouth

  • Hemorrhage

  • Irritability

  • Malaise

  • Muscle complaints

  • Edema

  • Seizure

  • Abnormal sensation

  • Sleep disorders

Pregnancy & Lactation:
Montelukast crosses the placenta following oral dosing in rats and rabbits. There are, however, no adequate and well-controlled studies in pregnant women. Because animal reproduction studies are not always predictive of human response, Montelukast should be used during pregnancy only if clearly needed. Because many drugs are excreted in human milk, caution should be exercised when Montelukast is given to a nursing woman.
