Indications:
Indicated for the management of essential hypertension, chronic stable angina, and certain supraventricular arrhythmias such as atrial fibrillation or atrial flutter.
Pharmacology:
Diltiazem is a nondihydropyridine calcium channel blocker that inhibits the influx of calcium ions into cardiac and vascular smooth muscle. It reduces myocardial oxygen demand and improves oxygen delivery to the heart by decreasing heart rate and relaxing blood vessels.
Dosage:
Adults: 30 mg 3 to 4 times daily before meals and at bedtime.
Dosage may be adjusted based on response and tolerance.
Administration:
Administer orally, before meals. Tablets should be swallowed whole with water.
Interaction:
May interact with beta-blockers, digoxin, statins, and CYP3A4 inhibitors. Caution with other antihypertensives due to additive blood pressure-lowering effects.
Contraindications:
Contraindicated in patients with sick sinus syndrome, second- or third-degree AV block (without pacemaker), severe hypotension, acute myocardial infarction, and pulmonary congestion.
Side Effects:
Common: Dizziness, headache, flushing, nausea, and bradycardia.
Rare: Edema, rash, constipation, and AV block.
Pregnancy & Lactation:
Use only if clearly necessary during pregnancy. Diltiazem is excreted in breast milk; caution advised during lactation.
Precautions & Warnings:
Monitor heart rate and blood pressure regularly. Use with caution in patients with hepatic or renal impairment and in the elderly.
