Indications:
Type 2 Diabetes Mellitus (T2DM) in adults
When treatment with Empagliflozin or Metformin alone does not provide adequate glycemic control
To reduce cardiovascular risks in diabetic patients with established cardiovascular disease
Pharmacology:
This fixed-dose combination contains:
Empagliflozin: A selective SGLT2 inhibitor that promotes urinary glucose excretion by inhibiting reabsorption of glucose in the kidneys.
Metformin: A biguanide that decreases hepatic glucose production and improves insulin sensitivity, thereby lowering blood glucose levels.
Dosage:
The usual starting dose is 5 mg Empagliflozin / 500 mg Metformin twice daily
Dose may be titrated based on patient’s glycemic control
Maximum dose: 25 mg Empagliflozin / 2000 mg Metformin per day
Adjust dose cautiously in patients with renal impairment
Administration:
Take orally twice daily with meals to reduce gastrointestinal side effects
Swallow tablets whole with water
Interaction:
Alcohol increases the risk of lactic acidosis with Metformin
Diuretics and other antihypertensives may enhance the risk of dehydration and hypotension
Avoid co-administration with cationic drugs (e.g., cimetidine) that compete for renal tubular transport with Metformin
Risk of hypoglycemia when used with insulin or insulin secretagogues
Contraindications:
Severe renal impairment or end-stage renal disease
Acute or chronic metabolic acidosis, including diabetic ketoacidosis
Hypersensitivity to either component
Type 1 diabetes
Side Effects:
Common: Nausea, diarrhea, urinary tract infections, genital mycotic infections
Rare: Lactic acidosis (Metformin), ketoacidosis (Empagliflozin), dehydration
Flatulence, abdominal discomfort, and dizziness
Pregnancy & Lactation:
Should be avoided during pregnancy unless clearly necessary
Not recommended during breastfeeding
Precautions & Warnings:
Monitor renal function before and during treatment
Avoid excessive alcohol consumption
Stop treatment in cases of dehydration, surgery, or serious infections
Educate patient about signs of lactic acidosis and ketoacidosis
