Indications:
Benign Prostatic Hyperplasia (BPH) in adult men
Pharmacology:
Silodosin is a selective antagonist of post-synaptic alpha-1 adrenoreceptors located in the human prostate, bladder base, bladder neck, prostatic capsule, and prostatic urethra. Blockade of these receptors causes smooth muscle relaxation in these tissues, leading to improved urine flow and reduced BPH symptoms.
Dosage:
Adults:
Normal Renal Function: 8 mg once daily with a meal.
Moderate Renal Impairment (CrCl 30-50 mL/min): 4 mg once daily with a meal.
Severe Renal Impairment (CrCl <30 mL/min): Contraindicated.
Administration:
Take the capsule with food.
Swallow the capsule whole with a glass of water; do not chew or crush.
Interaction:
Drug Interactions:
Co-administration with strong P-glycoprotein inhibitors (e.g., cyclosporine) may increase plasma Silodosin concentration.
Concurrent use with PDE5 inhibitors (e.g., sildenafil, tadalafil) and other alpha-blockers may lead to symptomatic hypotension.
Food Interactions:
Alcohol may increase the risk of dizziness and orthostatic hypotension; limit consumption.
Contraindications:
Hypersensitivity to Silodosin or any component of the formulation.
Severe renal impairment (CrCl <30 mL/min).
Severe hepatic impairment (Child-Pugh class C).
Concurrent use with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin, itraconazole, ritonavir).
Side Effects:
Common: Retrograde ejaculation, dizziness, diarrhea, orthostatic hypotension, headache, nasopharyngitis, nasal congestion.
Less common: Fatigue, nausea, abdominal pain.
Pregnancy & Lactation:
Pregnancy: Category B. Not indicated for use in women.
Lactation: Unknown if excreted in breast milk. Not indicated for use in women.
