Indications:
Hypertension (high blood pressure)
Chronic stable angina pectoris
Supraventricular arrhythmias such as atrial fibrillation and atrial flutter
Pharmacology:
Diltiazem SR is a nondihydropyridine calcium channel blocker that decreases calcium ion influx into cardiac and vascular smooth muscles, causing vasodilation and reducing heart rate and myocardial oxygen demand.
Dosage:
Adult dosage usually starts at 90 mg once daily. Dose may be adjusted according to patient response, up to 360 mg per day.
Administration:
Take orally once daily with or without food. Do not crush or chew the tablet. Swallow it whole with water.

Beta-blockers
Digoxin
CYP3A4 inhibitors or inducers
Other antihypertensives
Contraindications:
Severe hypotension
Sick sinus syndrome without pacemaker
Second or third-degree AV block
Acute myocardial infarction with pulmonary congestion
Allergy to Diltiazem
Side Effects:
Common: Headache, dizziness, flushing, edema, fatigue, bradycardia
Less common: Constipation, rash, nausea, liver enzyme elevation
Pregnancy & Lactation:
Use only if clearly needed during pregnancy. Excreted in breast milk; breastfeeding not recommended unless benefits outweigh risks.
Precautions & Warnings:
Monitor blood pressure and heart rate regularly. Use cautiously in patients with liver or kidney impairment and those with heart failure.
