Indications:

PRIMACAP 1000 MG is indicated for the treatment of:

  • Pneumonia and other respiratory tract infections

  • Urinary tract infections (UTIs)

  • Skin and soft tissue infections

  • Intra-abdominal infections

  • Febrile neutropenia (in patients with cancer undergoing chemotherapy)

Pharmacology:

Cefepime is a fourth-generation cephalosporin antibiotic. It works by inhibiting bacterial cell wall synthesis, leading to cell lysis and bacterial death. It is effective against a wide range of both Gram-positive and Gram-negative bacteria, making it a broad-spectrum antibiotic.

Dosage:

  • Adults and children over 12 years: The usual dose is 1-2 grams every 12 hours, depending on the severity of the infection.

  • The duration of treatment depends on the type and severity of the infection, usually ranging from 7 to 14 days.

  • Renal adjustment: Dose adjustments are necessary in patients with renal impairment.

Administration:

  • Administered intravenously (IV) or intramuscularly (IM).

  • For IV use, it should be diluted and infused over a period of 30 minutes to 1 hour.

  • For IM use, it should be injected deep into a large muscle.

Interaction:

  • Cefepime may interact with other antibiotics such as aminoglycosides, which can increase the risk of kidney damage.

  • It may also interact with anticoagulants like warfarin, increasing the risk of bleeding.

  • It can decrease the effectiveness of oral contraceptives.

Contraindications:

  • Hypersensitivity to cephalosporins or any component of the product

  • History of severe allergic reactions to penicillin or other beta-lactam antibiotics

  • Use with caution in patients with renal impairment or a history of gastrointestinal diseases (e.g., colitis).

Side Effects:

  • Common: Diarrhea, rash, nausea, vomiting, pain or redness at the injection site

  • Rare: Severe allergic reactions (anaphylaxis), seizures (especially in patients with renal dysfunction), blood disorders (e.g., thrombocytopenia)

  • Serious: Nephrotoxicity (kidney damage) in patients with renal impairment, Clostridium difficile-associated diarrhea

Pregnancy & Lactation:

  • Pregnancy: Category B – Cefepime is generally considered safe during pregnancy, but should be used only when absolutely necessary and prescribed by a doctor.

  • Lactation: Cefepime is excreted in breast milk in small amounts. It is generally considered safe during breastfeeding, but consult your healthcare provider for advice.
